Allograft and Biomaterials are highly technical products, and are regulated heavily by the FDA. In order to accept a return, we’re required to gather some information about the product, the clinician, and the circumstances.

To begin the Return Authorization process, please fill out the form and submit an RMA Request. We’ll review the information and respond as quickly as possible.


Return Process

1) Submit an RMA Request

2) Wait for a response from our team

3) If approved, we’ll provide an “RMA Number”. This RMA number is only valid for 30 days.

4) We’ll also provide an FDA-mandated form to fill out, if applicable. The dentist MUST sign this form for returns of allograft

5) Place the form inside the return shipment

6) Write the “RMA Number” on the outside of the shipping container

8) Once received, we’ll inspect the form and the products

9) We’ll issue a credit memo to the Authorized Dealer, or provide an exchange directly to the dentist
