Indications:
- Horizontal Ridge Augmentations
- Vertical Ridge Augmentations
- Extraction sockets
- Periodontal defects
- In conjunction with the placement of dental implants
Success Tips:
- Horizontal Ridge Augmentation – Tunneling Technique
- Can achieve up to 3-5mm of horizontal augmentation
- Vestibular incision must be made thru periosteum, completely to bone
- Tunnel must be 100% sub-periosteal (periosteum must be elevated intact, in order to shape tunnel and act as a membrane)
- Tunnel must be minimal in size to help immobilize sponge during healing
- Collagen membrane may be used. Collagen membrane is required if the periosteum is compromised.
- Bone must be decorticated to induce bleeding to the site
- Evaluate immobility of the sponge in the tunnel
- If immobile, then proceed to close the incision
- If mobile, then evaluate for fixation
- Can add particulate allograft around sponge to improve stability
- Horizontal Ridge Augmentation – Open Technique

- Must use collagen membrane
- Deep mattress suture may be used to hold membrane and sponge in place
- Primary closure is highly recommended. Not achieving primary closure requires expert training, evaluation, and certain parameters. Please call us for surgical consult
- Periosteal releasing incisions may be required in order to achieve primary closure

- Vertical Ridge Augmentation

- Excellent results can be achieved, but are highly variable, depending on patient health, compliance, and surgical technique
- Highly technique sensitive
- Requires fixation techniques
- Follows similar principles to the “Sausage Technique” popularized by Dr Istvan Urban

- Typically requires 6-9 months healing/remineralization before further treatment can be performed (verify calcification radiographically).
- Hydrate in sterile saline, or epi-free anesthetic, or blood. Hydrating with PRF is currently unstudied, and unproven.
- Sponge can be trimmed to size when hydrated, not when dry
- Ridge augmentation procedures are highly technique sensitive and require advanced surgical training from expert clinicians. Use without such training is not recommended and may lead to unsatisfactory outcomes. Clinicians must also understand the package IFU prior to use.
